After you leave your employer

You will lose your eligibility for membership in the Health Insurance Society after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.

Return your health insurance card when you leave your employer

Required documents:
  • Health insurance cards (for the insured person and all dependents)
  • Elderly benefits card (if issued)
Deadline: Within five days after the date of loss of eligibility
Applies to: Insured persons leaving employment and their dependents
Submit to: HR section of each corporation or company
Notes:  

If you wish to remain a member of the Health Insurance Society

Required documents:
Deadline: Within 20 days after the date of loss of eligibility of the insured person
Applies to: Insured persons who have been insured continuously for at least two months prior to leaving employment
Submit to: Health Insurance Society
Notes:  

If you lose your eligibility as a Voluntarily and Continuously Insured Person

Required documents:
Health insurance cards
  • Be sure to attach a copy of the new health insurance card from your new employer (the side of the card showing the date you became eligible). (This applies if you begin working for a new employer.)
Deadline:

Within five days after the date on which you lost your eligibility as a Voluntarily and Continuously Insured Person

Applies to: Voluntarily and Continuously Insured Persons who have become insured persons of another health insurance society or other insurer because they began working for a new employer
Submit to: Health Insurance Society
Notes: You do not need to submit notification and will automatically lose your eligibility in the following cases:
  • On the scheduled expiration date two years after becoming a Voluntarily and Continuously Insured Person
  • If you become eligible as an insured person under the Medical Care System for the Advanced Elderly (loss of eligibility on 75th birthday)
  • If you have not paid your insurance premiums by the date due
