Childbirth

“A Childbirth and Childcare Lump-Sum Grant” is paid when an insured person gives birth. “A Dependents' Childbirth and Childcare Lump-Sum Grant” is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

Request a Childbirth and Childcare Lump-Sum Grant

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Society. For details, check with the medical care institution or other facility where you plan to give birth.)
There is no need to apply for Additional Sum.(It will be processed after the claim arrives from the medical institution to the Health Insurance Society and pay the expenses about 3 months after birth.)

If you chose not to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and paid yourself (you made a payment directly to the medical care institution)

Submit the following application to the Health Insurance Society if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf.

Required documents:

Copies of agreement documents indicating the following:

  • The fact that you have not concluded a proxy agreement with the medical care institution on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions
  • The name of the AZSA Health Insurance Society as the insurer to which the application is addressed

Copies of receipts for and details of childbirth expenses indicating the following:

  • The fact that you are not using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions
  • Bearing a stamp certifying that the birth is eligible under the maternity medical care compensation scheme if you gave birth at a member institution
Deadline: As soon as possible
Applies to: Insured persons or dependents who have paid the entire amount of childbirth expense at the medical care institution without using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf
Submit to: Health Insurance Society
Notes:
  • Pay the entire amount of the childbirth expenses to the medical care institution up front.
  • After you submit an application, you will be reimbursed for the entire amount of the Childbirth and Childcare Lump-sum Grant/Additional Sum.

If you chose to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by medical institutions on your behalf (payment by the Health Insurance Society to the medical care institution)

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Society in advance.
(This option is available only for small medical care institutions that have submitted the relevant notice.)

Required documents:
A copy of your maternal and child health handbook or other document showing the expected delivery date
Deadline: In advance
Applies to: Insured persons and dependents expecting childbirth up to two months in the future who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf
Submit to: Health Insurance Society
Notes:
  • The Health Insurance Society will pay the entire amount of the Childbirth and Childcare Lump-sum Grant/Additional Sum to the medical care institution.
  • You are responsible for any amounts exceeding the Childbirth and Childcare Lump-sum Grant/Additional Sum.
  • If the expense is less than the Childbirth and Childcare Lump-sum Grant/Additional Sum, you will be paid the difference.
  • Since this system requires advance application, submit the required documents within the two-month period leading up to the expected delivery date.

When giving birth at an overseas hospital

Submit the following application to the Health Insurance Society if you have given birth overseas.

Required documents:

Birth certificate issued by the delivering medical care institution or public institution (original)
(No birth certificate is required if the physician or midwife has certified the Application for Payment of Childbirth and Childcare Lump-sum Grant.)

If the certificate is in a foreign language, a Japanese translation of its content (The translation must be signed by the translator.)
(If you translate the document yourself, be sure to sign the translation by hand or affix your official seal.)

Applies to: Insured persons and dependents who paid the entire amount of childbirth expense at a hospital overseas and used neither the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions nor the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on their behalf
Submit to: Health Insurance Society
Notes:
  • Pay the entire amount of the childbirth expense to the medical care institution up front.
  • After you submit an application, you will be reimbursed for the entire amount of the Childbirth and Childcare Lump-sum Grant/Additional Sum.
  • A Japanese translation of the certificate is required. Be sure to submit all required documents.

Add the child

After the child is born, take the necessary steps to add him or her as a dependent.
