Insured persons and dependents who wish to have an Eligibility Verification Certificate issued (reissued) for the following reasons:

  • Lost Individual Number Card
  • In the process of renewing Individual Number Card
  • Individual Number Card electronic certificate has expired
  • Individual Number Card issued but not registered for use as a health insurance card
  • No Individual Number Card issued
  • Individual Number Card returned
  • Support from a third party (e.g., caregiver) required to receive examinations and treatment with a Myna health insurance card
  • Lost or damaged Eligibility Verification Certificate

Applies to: Insured persons and dependents who wish to have the Notice of Eligibility Information reissued due to loss or damage

Notes: The [medical care insurance eligibility information screen] registered on Mynaportal may be substituted for the Notice of Eligibility Information. (You can also download this in advance.)

If you lose your health insurance card or Eligibility Verification Certificate

You may be asked to attach the following documents or document copies to verify the applicant’s identity.

  • An identification document showing a facial photograph of the applicant (e.g., driver’s license, passport). If the applicant does not have picture ID, an official certificate such as a certificate of residence.
Deadline: Immediately following loss of health insurance card
Applies to: Insured persons or dependents who have lost their health insurance cards or Eligibility Verification Certificate
Submit to: HR section of each corporation or company (for Voluntarily and Continuously Insured Persons to health insurance society directly)
Notes: The Health Insurance Society’s designated bank account appears in the middle section of the application form.

When there is no space left on your insurance card or Eligibility Verification Certificate

If there is no blank space available in the space for your address on the insurance card, use correction fluid or other means to remove your old address and write your new address.
While you are not required to notify the Health Insurance Society of a change in address, please go to the Cafeteria Plan Website and update it in the personal information section.
