When there is no space left on your insurance card or Eligibility Verification Certificate
If there is no blank space available in the space for your address on the insurance card, use correction fluid or other means to remove your old address and write your new address.
While you are not required to notify the Health Insurance Society of a change in address, please go to the Cafeteria Plan Website and update it in the personal information section.
