- *6 The company or government office where you are employed currently or were employed until recently (a copy of the employment agreement must be attached if you do not have three months’ salary details)

- *8 The postcard notice of pension remittance mailed from the Japan Pension Service around June of each year
- *9 Ward office or city hall for your permanent residence (In certain cases, a common-law marriage may be recognized to be in effect even if you have not officially wed. Check with the Health Insurance Society for details.)
- *10 Ward office or city hall for the place where you are a registered resident
- *11 The AZSA Health Insurance Society has established a minimum allowance amount (per person per year) for a dependent living apart: “Must be more than the dependent’s annual income; additionally, the total of the dependent’s annual income and the allowance must be at least 1.3 million yen.”
- *12 You may use documentation of the remittance. Note that you must conduct the remittance through a financial institution (such as bank, credit union, or post office) and submit a document showing the relevant information, including the name of the recipient and the amount and date of the remittance. Allowances handed to dependents in person are not acceptable because they cannot be documented.
- *13 AZSA Health Insurance Society has established a minimum allowance amount (per person per year) for a dependent living apart: “Must be more than the dependent’s annual income; additionally, the total of the dependent’s annual income and the allowance must be at least 1.3 million yen.”
